 Blackpool council’s home care and re-ablement service provides high quality care and support to people within their own homes and is accessible to people who have eligible needs identified through an assessment process in adult social care.

The service offers short term intermediate care and re-ablement and aims to develop goals in a collaborative way that optimizes independence and wellbeing, adopting a person-centered approach to service delivery whilst taking into account cultural difference and preferences. This approach aims to promote a better quality of life whilst supporting positive risk taking.

Our service helps to facilitate discharges from hospital and residential settings as part of the Discharge to Assess model and subscribes to a competency based trusted assessor approach. This approach enables individuals to be assessed within their home environment and identify any equipment that may be required to meet their needs.

We provide a range of services that also include palliative care, primary night care, rapid response and bridging care and work in partnership with a range of professionals to ensure a collaborative approach to our care delivery.

We have a skilled workforce that receive regular mandatory training to ensure that all our staff have the right knowledge, skills and abilities to undertake their role to a high standard. We provide tailored support in areas such as the administration of medication, personal care, meal preparation and domestic cleans, ensuring that the person is at the center of everything we do. We excel in meeting the requirements of individuals with complex and challenging care needs.

The home care and reablement service is regulated and inspected by the Care Quality Commission.
