Enablement Service

Enablement provides a rehabilitation and recovery service to help you to live as independently as possible at home following an illness.

We offer a service at the Assessment and Rehabilitation Centre (ARC), or Hoyle Resource Centre, in a nurse led unit or at home.

We can: 

  • Tailor support to your needs
  • Encourage you to learn new skills, or to re-learn any daily living skills you may have lost
  • Help to prevent unnecessary stays in hospital, or inappropriate admissions to hospitals or care homes
  • Build up your confidence to enable you to live independently at home
Enablement runs for a maximum of six weeks and is free of charge.  After six weeks if you still need help, you will be referred for a permanent package of care.

You will be given a financial assessment and you may have to pay for the services you receive.

We also run an Outreach Service that provides up to four weeks of support in the home when you leave hospital.  This is free of charge.

Social Workers, Hospital Discharge Co-ordinators, GP's, Community Matrons and the Nurse-Led Unit can contact the Enablement Service during normal working hours.
