If you go into Victoria Hospital or attend A & E and feel you need help or advice when you leave hospital, you can ask to see a Discharge Co-ordinator.
We can provide help, advice and information, and if necessary arrange for some short-term support at home. If it is necessary for the support you are receiving to continue, there will be a charge. There may be occasions when you have to leave home in an emergency, such as a sudden admission to hospital or a crisis admission to care.
If there is no-one left in the home, Social Services will take every step to make sure that your property and belongings are safe. This may involve contacting relatives, friends or neighbours for help.
It may also involve making emergency arrangements for the care and protection of your pets. If relatives, friends or neighbours cannot look after the pet, we will contact voluntary animal care agencies such as the RSPCA or we may make arrangements for care in a cattery or in kennels.
