Needs Assessment by Social Services

An assessment is a way of finding out about your care needs and what we can offer to help you. You will be involved in working out how your care needs can be met. This can be very simple and quick or take time depending on your needs or circumstances. If you look after someone you are entitled to an assessment of your own needs. This is called a Carer’s Assessment.

Before you have an assessment you should make a list of the things that you can and cannot do, who is helping you at the moment, what they do for you and anything else that is worrying you. If you write it down you can make sure that you cover everything. You can have someone with you during the assessment if you choose. This can be either a friend or relative or you can contact  Blackpool Advocacy (Tel: 01253 405959) if you want to have someone independent with you when you meet Social Services.
 
Your assessment will be done by someone from Social Services who is experienced in understanding care needs and how they can be met. Usually your assessment will be done in your own home. However, you can choose to come to the Social Services offices if you prefer. We are trying to find out if there is anything that puts you in danger, or at high risk.

You may be in danger if now or in the near future you have significant health problems, have been treated badly or neglected, cannot carry out personal care or household duties, or cannot carry out your family responsibilities.
 
You may be at risk if now or in the near future you are treated badly, cannot carry out most of your personal care or household duties, or cannot carry out most of your family responsibilities.
 
We aim to provide services to meet your care needs as described in your care plan within 28 days following your social care assessment although some care services may take longer to arrange.
 
We will keep you informed of progress, notify you as soon as arrangements are in place and send you a copy of your care plan.

Sharing Information about you

During your assessment we will make a note of information about you such as your name, address, date of birth, next of kin and details about any disabilities or illnesses. We will keep your information safe and secure.

We often need to share information about you with others involved in your care, such as a hospital consultant, district nurse, health professional or other council department. We will assume that we have your consent to do this unless you tell us not to.

Have you ever had the feeling that someone somewhere in Blackpool knows what you want to know – if only you knew who they were? Blackpool Signpost Service can help to put you in touch with services and organisations throughout Blackpool.

If you tell us what you want to know we will find the information you need and pass it on to you. You can use Blackpool Signpost Service if you:

· are over 18 years of age;
· live in Blackpool.

You can either fill the form in yourself or ask someone to do it for you. The more information you give us the better the information we will be able to give you. The important thing is that the form is all about you and what you want to know. If anyone else wants to find out about things they should complete a form of their own.
