What questions will you ask when I register the birth of my baby?

| 3. What are the first names and surname of your baby? |

| 4. What are the first names and surnames of the baby's parents? |
|
| 5. Has either parent ever changed their name or used any other, including Maiden name (name prior to marriage)? |

| 6. Where were the parents born (village, town or city or district)? |
|
| 7. What are the parents' present occupations? |
|
| 8. What is the present address of both parents? |
|
9. What are both parents' dates of birth and, if applicable, date of marriage? (This information is legally required but is not entered in the register). |

It is very important that the information recorded in the birth register is correct. If any mistake is made, for example in the spelling of a name or surname or in the description of the parents’ occupations or places of birth, it will give the parents or other person who registered the birth some trouble to have it put right.
The person registering the birth should check the information recorded in the register very carefully before the entry is signed.
