Where do I send the BD8 form which is issued at the death registration?

You can either
  • send the BD8 form to the Department for Work and Pensions (DWP)
    After completing the reverse of the white BD8 form, it should be sent to the appropriate office to ensure that everyone’s records are kept up to date, even if you or they are not claiming any benefits or receiving a pension.

    For people who die before they are aged 60, the form should be sent to:
    Job Centre Plus
    Barry House
    London Road
    PRESTON
    PR1 4DE

    For people who die after the age of 60, the form should be sent to:
    The Pension Service
    PO Box 680
    WARRINGTON
    WA1 1AE

  • or you can telephone the DWP Bereavement Services line on 0845 606 0265
    The staff may answer the phone saying "Pension Services" but this is the contact number for specially trained Bereavement staff.
