Supporting Young People Leaving Care

Our Leaving Care team work to improve the life chances of young people living in and leaving Local Authority care.   We provide support and advice to young people between the ages of 15 and 21 - 24 years if in Higher Education.

The main aims of our team are to:
  • prepare young people looked after by the Local Authority for the time when they cease to be looked after, and the provision of after care advice and assistance
  • support young care leavers through the transition from being looked after to independent living
  • improve life chances of looked after young people.
Each young person is allocated a Personal Advisor to develop and implement a Pathway Plan which aims to identify any support required in the following areas:
  • health
  • practical skills
  • accommodation
  • finance
  • personal life skills
  • education, training and employment.
Each Personal Advisor keeps in regular contact with their young people and drop-in sessions are held at Church Street during the week to allow the young person to meet and get support and guidance from their Advisor.
We work closely with other agencies, providing links with Housing, Connexions, Young People's Health and Information Centre, Community Drug Team, Youth Offending Team and other departments within Social Services.

We also offer a drop-in service for young people at the town centre office at 307 - 309 Church Street. 

Our Emergency Duty Team can be contacted on 01253 477600 after 5pm.

The team works within the guidelines of the Leaving Care Act 2000.
