The review is an opportunity to make plans and decisions which will be very important to the child/young person you are caring for.
As their carer, it is important that you contribute to their review and your views are taken into account so we can make the best plans possible.
Please take some time to think about what you think is going well and what further support or changes to their care plan if any would improve things for them.
