Referrals for Speech and language therapy

Referrals for the Neuro Developmental Pathway

  • Referrals should be made using the School-Age ND Pathway referral form. Referrals must be made with the knowledge and consent of the person(s) with parental responsibility for the child or young person
  • The service does not accept self-referrals. The child or young person being referred must have a GP in Blackpool Fylde or Wyre, and evidence of targeted interventions already in place must be detailed in the referral
  • All referrals go to the neurodevelopmental pathway clinical lead and will be triaged with one of the pathway consultants. If accepted the pathway coordinator will contact the families to discuss the process and to start gathering further information about the child or young person prior to their assessment with one of the 6 pathway consultants
  • A referral can be made by an involved professional, or via a school teacher/SENDCO

Referrals for Blenheim

  • The referral will then be triaged by the multidisciplinary team and a timeframe allocated for an appointment. Some children will need to be seen urgently and this is decided at triage so please include as much detail as possible about the child's strengths and areas of need

Referral for Options4CYP (Options4CYP is the umbrella name for the NHS mental health support team)

Before making a referral into our Options4CYP Single Point of Access (SPoA), please consider

  • How the school environment, via a whole school approach to emotional wellbeing, can be used to offer support to the child/young person
  • How pastoral support within school can be used to support to the child/young person
  • Discussing the needs of the child/young person with our PMHWs or, if you are from a secondary, special, appropriate provision or sixth form/college setting, with a member of staff from our MHST
  • Discussing the needs of the child/young person with your link advisor from Blackpool's SEND support team
  • Most of us know when we are mentally and physically well, but sometimes we need a little extra support to keep well. There are five simple steps that may help maintain and improve your well-being. Try to build these into your daily life – think of them as your ‘five a day’ for wellbeing.   • Encouraging the child, young person and their family to access one of our PMHW drop ins - run weekly which are published on the SEND Events calendar latest events in SEND

If all the above have been completed, please contact our PMHWs who will support you to make a referral to our Options4CYP Single Point of Access (SPoA). Referrals are screened by a multi-disciplinary team every morning, Monday to Friday, and may be allocated within Options4CYP or signposted to other community services

  • Referrals will not be accepted from parents/carers or educational settings
  • The paediatric physiotherapy service accepts referrals from the following professionals: Doctors, consultants, allied health professionals, health visitors

Referrals for occupational therapy

  • Referrals will not be accepted from parents/carers or educational settings
  • The paediatric occupational therapy service accepts referrals from the following professionals: doctors, allied health professionals, ward sisters, specialist nursing, the children’s community nursing team, health visitors and ‘Single Point of Entry’ referrals or social worker (home assessment only)
