The reablement team will support you to regain or make the most of the skills and support you already have. You may have lost your confidence as a result of a fall or following a planned operation and just need your confidence re-building. Our specialist team will help you with your day to day tasks and find ways, where possible, for you to do things safely for yourself. The length of time we provide this support is up to six weeks.
This period of support will also enable us to determine if you require any ongoing support after the Reablement ends. We may suggest a range of other support or services which may help to increase your independence for example, advice to maximise your benefits, equipment from an occupational therapist, local assisted shopping services, or information about accessing social activities and support within your local community.
We will be in regular contact with you and the teablement team during this period and the support you receive may increase or decrease depending on your progress.
We will aim to
- Tailor support to your skills and abilities
- Encourage you to learn new skills, or re-learn any daily living skills you may have lost
- Prevent unnecessary stays in hospital or a care home
- Build your confidence enabling you to live independently at home
- Identify and reduce any risks to your safety where possible
How much will I pay?
There is no charge for reablement services. The reablement period is up to six weeks, the duration may be less depending your progress and your potential for rehabilitation, for example, if you have reached your goals before this time the reablement will finish at this point.
We will offer you a financial assessment if you need ongoing care after the reablement period has finished. Charges apply for most services provided by Blackpool social services and are applicable from the day services start. Some people pay a reduced amount or don’t have to pay anything at all. We will offer to carry out a financial assessment once you have completed and returned an application for a reduction in charges telling us about your income, savings and expenses.
Sharing your information
We will record information about you such as your name, address, date of birth, legal status, next of kin and details about your income, expenditure, savings and capital plus details of your circumstances gathered at the assessment.
We will keep your information safe and secure. Sometimes we may need to share information about you with others involved in organising or delivering your support, such as care agencies, advocates, health professionals or other council departments. You should let us know if you do not want us to do this.
